Breast augmentation is the use of surgical implants to improve the shape, fullness, size and appearance of the breasts. Dr. Nealy believes that breast augmentation cases, like any of our other cosmetic procedures, should be unique to each patient. What type of Breast implants are used?

The type and shape of implant is as important to the outcome of the procedure as the artistry and skill of the surgeon is. At Silk Touch, Dr. Jason Nealy prefers to use Allergan Natrelle round smooth implants. Allergan has approval for 3 types of silicone gel fillers: Responsive silicone gel, SoftTouch silicone gel, and Highly Cohesive silicone gel. Ranging from soft to firm is as follows: Responsive < Soft Touch < Cohesive.

Textured implants are rarely used due to a very rare associated lymphoma called Breast Implant Associated Anaplastic Large Cell Lymphoma (BIA-ALCL) and are not used at our practice. BIA-ALCL is typically found near the breast implants fibrous scar capsule and not in the breast tissue itself. Patients who currently have textured implants do not need these taken out emergently but should be aware of the rare occurrence of ALCL and early symptoms.

“Gummy Bear” implants have received great media attention since the mid-2000’s. These were classically shaped, highly-cohesive, textured implants. The same name is now being used on all generations of implants due to the improved silicone cohesivity. This cohesivity allows the implant to be cut in half without the gel spilling out (known as form-stable).

Saline implants use a silicone shell filled with salt water. The advantage of a saline implant is that they can be placed through a smaller incision and can be filled differentially. This means modest breast asymmetries can be corrected by filling one side more or less than the offer. Unfortunately, saline implants tend to show more rippling deformity and are more likely to rupture compared to silicone gel implants. Silicone gel is a more popular choice due to the more “natural” feel. Silicone better mimics native breast tissue in feel and appearance.

Our procedures are customized to each patient. Dr. Nealy will work with the patient’s desired outcome and their unique anatomy to choose the right implant for them. We have model implants for patients to touch and a unique sizing system to demonstrate how their implants will look on them.

Breast Augmentation FAQ:

Yes! We believe that patients should be involved in the design of their breast augmentation, and that includes incision placement. Many options exist for breast implant surgery including the inframammary fold (lower breast crease), periareolar (around the lower border of the areola), through the axilla (armpit), and through the umbilicus (belly button). Each incision location has advantages and disadvantages – Dr. Nealy will go over and recommend incision placement based on your anatomy and goals.

While recovery after breast augmentation can vary slightly from person to person, most patients are back to work in 4-6 days.

At Silk Touch, Dr. Nealy employs a very active post-op regimen for breast augmentation surgery. In addition to pain management patients should be walking on the first day of surgery for 30 minutes and continuing this three times daily thereafter. Breasts are supported with a sports bra for six weeks.

Implant placement will depend on the patient anatomy, medical history and aesthetic goals. There are six potential places for the breast implant to be placed. These include below the breast gland and above the fascia and pectoral muscle; below the pectoralis fascia; completely below the pectoralis muscle; or in a dual plane, meaning below the muscle in the upper pole and below the gland in the lower pole of the breast. There are three types of dual-plane placements which vary in the amount of implant is covered below the muscle.

breast augmentation pink1 200x300 1 In retail, breast size is given in cup sizes (A,B,C, and so on.,) however implants are measured in cc’s, or cubic centimeters. Generally, 150-200cc is required to increase one cup size. Each patient is different and it will vary widely on how much you need. The best way to determine size is to schedule a consultation for sizing which involves breast measurement and trying on implants with a specialized bra.

First and foremost–breast asymmetry is absolutely the norm, not the exception. Most women have asymmetric breasts, either from glandular development as a teen or from asymmetric milk production and involutional changes associated with pregnancy. For some, this is apparent and causes the patient to feel self-conscious. But the good news is this is absolutely able to be corrected using implants. Asymmetry can be improved by using differential sizing during breast augmentation surgery.

Scarring is an inevitable part of breast surgery but can be hidden in natural skin creases and kept small so they are unnoticed. Scar surveillance will take place during postoperative visits to make sure healing is progressing well and scars are behaving. Topical medications and creams in combination with our state of the art laser treatments can be used to make most scars nearly invisible after one year.
